Twilight Team visits are more than just routine check-ins — they’re intentional, nurse-led visits designed to help your loved one end the day safely and with dignity.

60-Minute Nursing Visit

Evening care with clinical awareness, delivered by experienced RNs.
This foundational visit supports one individual through calm, hands-on evening routines—while offering the added value of professional nursing observation. Ideal for clients who benefit from consistent support, wellness monitoring, and a trusted presence who knows what to watch for.

Each visit may include:

  • A full home safety and environment check (locks, lights, mobility hazards, medical devices, bedside setup)

  • Assistance with evening routines (pajamas, hygiene, toileting, oral care, positioning, and comfort)

  • Medication reminders or handoff from a pre-filled planner or blister pack

  • Vital signs, as requested or aligned with existing care preferences

  • RN-led observations related to skin integrity, orientation, breathing, or behavioral cues

  • Notes on any visible symptoms or medication effects for caregiver/provider follow-up

  • Meaningful companionship and emotional support from a consistent, trusted nurse

  • A written visit summary delivered to the family after each visit

90-Minute Nursing Visit

Expanded support for clients with complex needs, chronic conditions, or recent health changes.
This extended visit includes everything offered in our 60-minute service, with additional time and focus for individuals who benefit from closer observation, mobility assistance, or emotional support.

Each visit may include:

  • Extra time for fall prevention or safe mobility routines

  • Additional vitals or wellness checks (e.g., blood pressure, oxygen levels, blood glucose readings using client devices, weight tracking, or observation of swelling or fluid retention)

  • Ongoing observation of how the client is tolerating medications or routines

  • Enhanced communication with families to support timely action and coordination

  • Basic supportive care tasks within RN scope, including:

    • Routine catheter bag emptying and hygiene based on existing instructions (not insertions or device troubleshooting)

    • Supportive ostomy pouch care aligned with the client’s established routine (not initiating new appliances or altering skin barriers)

    • Reinforcement of clean, non-medicated wound dressings and observation of skin or wound changes

  • Extra space for emotional presence, especially during periods of distress, behavioral symptoms, or cognitive changes

Ideal for clients recently discharged from care, managing long-term conditions, or those whose needs vary from day to day. Our experienced RNs bring steady, skilled presence to support both physical comfort and emotional wellbeing.

Medication Support

Peace of mind, one dose at a time.
Medication routines can be stressful. Our optional weekly med planning add-on helps families stay organized and reduces the risk of missed or duplicated doses.

Includes:

  • Weekly pillbox setup by a licensed RN

  • Tracking of prescription changes, refill timing, or discrepancies

  • Quick updates if we notice anything that needs attention

  • Adds 15 minutes to the visit time to support pillbox setup

Perfect for families managing care remotely or for clients with multiple evening medications.

Support for Two

Dual care without compromise.
For households where both individuals benefit from support, we offer a flexible add-on for shared visits.

Includes:

  • Everything included in the 60-minute visit, tailored for each person

  • Separate documentation and follow-up notes

  • A respectful and efficient way to support couples or cohabitating family members

  • Adds 20 minutes to the visit time to support a second client
